Philosophy of the NHTD Waiver:

The NHTD Medicaid waiver works on the principle that individuals with disabilities and seniors have equal rights to self-determination, risk-taking, and learning from life experiences. At the same time, the program holds the responsibility of ensuring the health and well-being of its participants.

Waiver services are tailored to the unique strengths, needs, preferences, and goals of each participant. The individual takes the lead in decision-making and collaborates with providers to develop a Service Plan. This process promotes personal empowerment, enhances independence, fosters community integration, cultivates self-reliance, and facilitates meaningful, productive activities. The satisfaction of NHTD Waiver participants serves as a significant indicator of the program’s success.

What is the NHTD Waiver Program?

The NHTD waiver program is a home and community-based program that helps New York´s Medicaid-eligible seniors and people with physical disabilities receive the comprehensive services they need while they live in a community-based setting rather than in a nursing home, congregate care setting, or other institution.

NHTD Waiver Eligibility:

  • Be eligible for nursing facility level of care
  • Be a Medicaid recipient
  • Be 18 years or older if, between 18-64, documentation of a physical disability is required
  • Choose to live in the community rather than a nursing facility
  • Have or find a living arrangement that meets the individual’s needs
