The NHTD service coordinator will assist the applicants and participants in obtaining and coordinating the services that are necessary to remain in or return to their homes.
Our Service Coordinator will serve as the primary contact for the participant in the Waiver Program and will provide ongoing assessment and reevaluation of the participant’s health and welfare and a determination of what services are needed to maintain this individual within their home.
